My shoulder injury started 9 months ago from weightlifting. There wasn't any one particular event rather a gradual increase in pain while working out and a feeling of instability . My pain gradually worsened and external range of motion and abduction decreased along with grinding , popping out sensations and impingement. I started pt 3 months ago and while I made improvements in winging and RC strength my internal rotation suddenly got worse, MRI shows a small tear in posterior labrum and a new pt says I now have posterior capsule tightness. The os gave me 2 cortisone injections with minimal improvement. He says I should have surgery if I want to remain active . My new pt says that the previous pt should have mobilized the shoulder and that I should mobilize it before considering surgery. While that makes sense, I am concerned that if she does improve the range of motion, that when I start working out I may tear it further and have flare ups and just prolong the inevitable. Any opinions out there on whether I should just go ahead and do surgery?

Surgery does NOT always secure a better outcome....sometimes a shoulder that intermittently pisses you off is better than a shoulder that (post-op) doesn't work for your life style at all...
3/51 Bilateral VIII C.N. complete neurectomy
2/60 Failed Supratentorial electrotherapy w/subsequent pancerebral resection
2/89 Bilateral aqueous humor drainage w/complete retinectomy
3/02 Quadralateral proximal extremity resection from gestational thalidomide exposure

Thanks Cavitator, I think you may be right. One week of mobilization is helping. PT found that my serratus and subscapularis were adhered together and my subscap was really tight-I pulled the serratus during a bench press when all this stareted. After 1 mobilization, I can now reach behind my back and is almost as much ROM as the normal arm. My external ROM is improving too and no pain and improving. Thing thats scary is I wouldve done surgery, that wouldnt have fixed the muscular issues. Amazing what a good PT can do. The other PT may have further inflammed the joint aggressively trying to stabilize it when all along i had a tight subscap pulling everything out of whack. Makes you wonder that everyone here that has issues several months post op needs to find a PT that is good with manual therapy.
