I recently had a MRI/MRA on my shoulder for what my doctor called a labrum tear, the radiologist who performed the athrogram portion of my MRA said they were looking for a slap tear. I looked more into this and found out some information and this forum.

Anyways my pain started about a month ago, I had recently started working out 4-5 days a week after living a somewhat seditary lifestyle for the past 4 years mainly playing pc games. I work in a enviroment that is mainly reptitive movements, and playing games also have caused shoulder issues for me in the past. Mainly rotator cuff impingements.
1 month ago I was working out and began to feel some imflammation in my shoulder, I didn't really stop to think much of it until I went to military press 135lbs and felt something in my right shoulder/neck area give out and felt immense pain. I went to a orthapedic doctor walk in clinic and they said i had some rotator cuff tendonitis.. did some PT sessions and started feeling better aside from some lingering pain. I had done rotator cuff rehab before and these sessions with PT reall didn't do much for me.. they never tired my shoulder or even made it feel sore. Anyways the pain I was expericing got really weird, it went from in front of my shoulder joint to mainly around my scaupula and still ocassaionally some pain the joint. I experience numbness in my hand too.. I had a follow up with a actual orthapedic dr that my family has used for r/c surgery and a mencusis tear before. He was the one who said I have a possible tear in my labrum. I looked up the slap tear and found that a common physical test is a o briens test, which mmy DR used and I know that when he did that it hurt TERRIBLY and when he cranked it around it really didn't feel that great.

The pain I experience is overall stiffness in my shoulder, and a dull aching pain that sometimes feels like its in my front shoulder area to my scaupula.

I've had a hard time sleeping since I had my MRA, it took 48 hours for the joint discomfort to go away and on tuesday I find the results..]]>

when i feel and look at my shoulders.. they DEF looked uneven, the shoulder that got operated on looks like its curved in more

I had bankart surgery done about three weeks ago. I'm afraid that I re-injured it when I was playing ultimate frisbee. I jumped and had both my arms overhead and as soon as my right arm began to come down, I heard two pops and felt immediate pain where my initial tear was. The pain went away within three minutes and feel almost no pain about an hour later. Strange thing is that I have a better range of motion with a little pain near the end of my range of motion. Is this some miracle work or did I tear something again?

I am able to do a decent casual weightlifting gym routine, including dips, pullups and pushups, and other exercises such as bench press, curls, etc. The shoulder feels stable during these exercises and pain-free while I am doing these exercises (I originally tore it doing a chinup). My range of motion is mostly back, although I still have a little trouble putting my arm behind my back to touch my shoulder blade. I have two main issues: (1) I still have difficulty with "weird" movements, such as 90 degree slow rotations as if I was going to throw something - I get a little shot of discomfort in my upper arm near my elbow and my forearm, and (2) I get discomfort at the extreme end of ROM when I lie down with my back on the floor and lay my arm flat above my head - I feel it in my upper pec and front of my shoulder under my delt near my chest. Occasionally if I work out hard, I get a little tingly feeling in my pinky and part of my palm, but it goes away if I don't work out for a few days afterwards, and I can still feel everything and move everything. It will be a little sore in the front of my shoulder.

I asked my PT and she said it sounded like residual scar tissue pressing on some nerves in the front of my shoulder. She did not seem concerned and thought that we could do a few sessions if it doesn't get better by the next time I see the ortho. My orthopeadic sports surgeon didn't seem concerned, and he is familiar with my regimen. I see him again in a month, and he saw me 6 weeks ago and several times before that. I am really hoping that I do not have to get a cleanup surgery or another fixit surgery. This recovery was very hard for me. I tightened up immediately after surgery and could not stand up straight for weeks. I had biceps tendon inflammation early in PT and it set me back. Then I had a very tight posterior capsule that further delayed me because my shoulder kept fighting to protect itself. Stretching PT was so painful that I had trouble sleeping the night before, as I dreaded it. It has been a long road and I outright dread doing it again or having worse complications. I have tried to be conservative in increasing the weight at the gym.

Anyway, I am going to try to do a little treading water in a small pool that I have access to and see if that helps. Has anyone had these issues? Any suggestions?]]>

getting sharp pain/discomfort in the lower outside of my bicep couple inches above my elbow

Rotator cuff: There is mild bursal surface fraying of the
posterior fibers of the supraspinatus tendon. Intermediate
signal and mild thickening of supraspinatus tendon fibers are
consistent with tendinosis (series 8, image 16). Infraspinatus,
teres minor, and subscapularis tendons are intact. No rotator
cuff muscle atrophy or fatty infiltration is present.

Labrum: There is a SLAP type labral tear involving the superior
labrum extending to the posterior-superior, posterior and
posterior-inferior labrum, as well as involving the biceps-labral
anchor (series 8, image 16). Anterior and posterior labrum has a
blunted morphology in keeping with degeneration. Focal contrast
pooling adjacent to the labrum within the rotator interval
measuring approximately 0.7 cm may represent a paralabral cyst
versus focal loculated fluid (series 8, image 21).

Ligament capsule: Intact. Mild synovial thickening is
demonstrated.

Biceps: There is linear signal irregularity within the biceps
anchor, as previously noted, likely representing extension of
SLAP tear to the biceps anchor. Remaining portions of the long
head of biceps tendon are intact.

Bones/cartilage: Mild focal marrow edema signal is seen within
the region of the greater tuberosity posteriorly, nonspecific. A
focal near full thickness cartilage defect along the superomedial
humeral head measures 0.9 cm TRV. Cartilage signal heterogeneity
is seen along the glenoid, with mild thinning noted posteriorly.

Acromioclavicular joint: There is moderate acromioclavicular
hypertrophy with associated subchondral edema. A small
subacromial spur measures 0.6 cm (AP).

Miscellaneous: There is no significant subacromial/subdeltoid
bursal fluid accumulation.

IMPRESSION:
1. SLAP-type labral tear extending from the superior labrum to
the posterior-inferior labrum with involvement of the biceps
anchor, as above, with suspected subcentimeter focus of contrast
pooling with the rotator interval, which may represent focal
loculated fluid versus a paralabral cyst, as above.
2. Mild tendinosis and bursal surface fraying of the
supraspinatus tendon without discrete tear.
3. Subcentimeter near full thickness cartilage defect along the
humeral head.
4. Moderate acromioclavicular arthropathy.
5. Synovial thickening compatible with mild synovitis.

I discussed these findings and imagery with Dr. Lawrence Higgens at Brigham & Women's hospital in Boston earlier this week. While I seem to pass all physical exams for SLAP tear, the MRI results would seem to say otherwise. Dr. Higgins was really on the fence in terms of surgery since I have very good strength across all physical tests.

I've now had my 2nd cortizone shot and plan to keep rehabbing over the summer. I'm also planning on getting other opinions.

In short, I'd be curious what others think about these findings and if I could get a gauge on severity based on the above. I clearly have pain, but it's not debilitating. I'm also curious if there are specific rehab exercises that could help me avoid surgery.
